3. Undertaking/Declaration Form
4. Birth Certificate/Baptismal/Ghana Card(Photocopy)
5. National Health Insurance Card (Photocopy)
6. Enrolment Form (Endorsed by JHS Headteacher)
7. Two (2) recent passport size photographs
